Insurance
I am an out-of-network provider. Services are paid for, in full, at the time of the appointment. Insurance benefit claim forms, if requested, are sent by the client to the health insurance company with appropriate codes for reimbursement.

Services may be covered in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What are the benefits for out-of-network providers?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Payment
Cash, check, money orders, debit cards and all major credit cards accepted for payment.

Cancellation Policy
There is a 24 hour cancellation policy in effect.
